Our residents never have to leave home for their health appointments, which is incredibly important for those who suffer from a severe mental health diagnosis.

Thanks to our partnership, residents can rely on quality care coordination in our Communities, whether they need durable medical equipment, labs, behavioral health services, or more.

Getting Primary Care – We proudly partner with Twin Cities Physicians, who specialize in providing quality care to senior adults living in our communities. Our residents never have to leave home for their health appointments, which is incredibly important for those with behavioral issues that can occur in stressful situations.
We Accept Residents Under the Age of 55 – At Sunflower Communities, we accept CADI, EW & BI waivers. Our residents do not need to be 55+, and we welcome the opportunity to assist you with your less than 55 YO referral needs.

Frequently Asked Questions
In addition to primary care, therapy, and care coordination services, residents can also rely on our pharmacy services. Prescription medications can be delivered right to the Community so our residents never have to take a trip to the pharmacy.
We understand the importance of pets and their role in mental health therapy, and we also know that pets are family. That’s why we proudly accept pets at our Elmore Community (dogs & cats/one pet per person). Details and fee information can be found here. We also offer complimentary pet care for our residents in the event they become unable to care for their pets due to a change in their physical or mental ability.
Yes! Both of our Communities provide secure memory care. We are proud to offer four different care levels to accommodate a continuum of care so our residents can stay at our Community and transition to memory care if/when needed.
Yes! Both of our Communities allow cigarette smoking outside in a secure setting; both of our Communities are both alcohol and smoking free within the buildings themselves. We will also provide assistance for those residents who are on a smoking plan by providing a secure location to maintain their cigarettes and disbursing them per their smoking schedule.
